Geriatrics Lead

Dr. Sztramko is a Geriatrician and Internist who completed his medical training at the University of Toronto, UBC, and the University of California. 

He was previously an Assistant Professor at McMaster University and Associate Program Director in the Core Medicine training program at UBC.  

He has founded an online education tool called www.igericare.ca to help caregivers of patients with dementia improve caregiver self-efficacy, which has been funded by patient groups including the Alzheimer’s Society.

Dr. Palombo is a Family Physician with over a decade of clinical experience providing care of the elderly and palliative care in patients' homes. She is passionate about helping seniors age well in place at home. Dr. Palombo is an Associate Clinical Professor at McMaster University and TMU School of Medicine, where she acts as the Family Medicine Director of Sites and Wellness as well. She is also a lecturer at the University of Toronto where she enjoys teaching medical learners at all levels of training.

Associate Physician

Dr. Niu is an Internist who completed her medical training at Western University of Health Sciences in Southern California, and her Internal Medicine residency and Geriatrics fellowship at Cleveland Clinic. She employs the 5M model (mentation, co-morbidities, medication, mobility and what matters the most) to provide comprehensive geriatric care that aligns with a patient's values and goals. She also enjoys providing education about healthy aging to her patients and their family members, and using non-pharmacologic tools to preserve and enhance their quality of life.

Dr. Olivia Geen is a specialist in internal and geriatric medicine and an adjunct Assistant Professor at the University of Toronto. She has a Masters in Translational Health Sciences from the University of Oxford and is the CEO and Founder of Aldora Health, a health education platform that curates leading research on better brain health. She is passionate about dementia prevention and memory improvement, creating personalized lifestyle and medication treatment plans for her patients.

Geriatric Nurse Clinician

 Lisa is a Nurse Clinician that has 36 years of nursing experience, with the last 14 years spent specializing in Geriatrics. She is a graduate of the University of Windsor with a Bachelor of Science and Masters of Science in Nursing. Lisa is passionate about the holistic care of Geriatric patients and their families, with an emphasis on helping patients maintain their quality of life and goals of care in their own home.

Medical Office Assistant

Catherine is a Registered Nurse with six years of nursing experience and over 24 years of medical administrative experience. She is passionate about community nursing where she can provide patient centered care for individuals from the comfort of their home. She is a graduate of Humber College and the University of New Brunswick, where she obtained her in degree in nursing.
