We know that one of the worst parts about feeling unwell can be the wait to find out what is going on. When it comes to memory & aging, seeing a Geriatrician in Ontario (Canada's specialized memory & aging doctors) normally takes anywhere from 6 months to 2 years.
At Ontario Virtual Memory Clinic, we've found a way to improve access to a comprehensive memory & aging assessment through interdisciplinary care teams so that you can be seen in as little as 1 month.
Our team includes geriatricians, family physicians, physician assistants, and nurses, all working together to figure out who you are, what matters most to you, and what we can do to help. By recognizing the skills of each team member, we can all practice to our fullest potential.
With our innovative model we are able to see you sooner to provide a diagnosis and create a personalized treatment plan for you or your loved one.
No more waiting. No more wondering.
You will get a call from our nurse specialist to gather information on your health & book your first appointment. You may also receive a call from a pharmacist to review your current medications, which ensures our team has the most up-to-date information. By the time you meet the doctor, they'll have a head start on understanding what matters most to you.
At your first virtual visit you will be assessed by a doctor or physician assistant, much like a typical visit to your family doctor - but from your home!
We usually spend 90 minutes with each patient and their family.
Depending on your clinical needs, you will meet with either a geriatrician or family physician with extensive experience in the care of older adults.
They will review your testing, answer your questions, and create a personalized treatment plan.
From the moment of your first appointment, you have access to our whole team for ongoing telephone support and management when you need it, from the convenience of your own home.
Our innovative approach puts people first - no more wondering, no more waiting.
We're here with you every step of the way.
We want to give you the best care possible, so it's important to us that you know what to expect.
We are a fully virtual clinic so that we can help people like you all over Ontario. If our team feels you need to be seen in person, we will do everything possible to connect you with resources in your area, including direct communication with your family doctor.
Like most clinics, we use an electronic medical record with a built-in telemedicine feature. All you need is an email address and internet connection. We'll send you a link to your appointment. Simply click the link, and meet the team.
Our nurse will go over everything with you during their first phone call with you, but in general, we collect information on:
1. Your current medications
2. Your medical history (usually provided to us by your family doctor)
3. Important life details about you
4. Your symptoms
Because many of our patients come to us with concerns about their memory, we also ask that someone you trust also attends part or all of the appointment, to fill in any gaps in the information. This is often a spouse, child, friend, or other relative.
All we need to get started is a referral from your family doctor. They can visit our Referral page for more details.
