We offer a team-based approach to the care of older adults, including:

• Evaluation of memory concerns or cognitive symptoms, including confirmation of diagnosis of dementia 
• Assessment of driving safety in patients with cognitive concerns 
• Forms/applications for insurance, disability, tax purposes (private charges may apply)
• Polypharmacy & deprescribing management
• Caregiver supports and linkages with appropriate community agencies

Our team consists of specialist geriatricians, primary care/family physicians, advanced-practice nurse and support staff.  There is no negation for physicians in rostered care models. 

There are no LHIN/catchment area boundaries for this service; as long as the patient is in Ontario and has OHIP, they can be seen.
